The problem
Follow-up for heart failure and hypertension patients after discharge ran on blood pressure and pulse readings written into a notebook at home. When a patient forgot a reading or wrote it down wrong, the doctor could not settle the right dose. The real risk, though, was that intermittent rhythm problems do not show up on the short ECG taken in hospital, so a crisis was usually noticed only when the patient reached A&E.
